Does DM affect VA?
Is VA affective for a single tank with a large ToJ? Why or why not?
Is VA effective during melee combat, ranged combat or both?
Your opponent's DM, yeah.
Sometimes, it's not normally cost effective until you do 10k damage or more per hit though, also it'd probably end up being a waste of a spell slot once your minions start being more powerful than your tat.
