To-Hit Calculation (in General)

Did anyone know how the to-hit calculation works? I did try looking through old changelog, but end up just as confused.

so CTH = Min (PTH from weapon + weapon's base +/- DX advantage - evasion and DB, weapon's base) - 20 if there is AoI
is that correct?

the PTH of elbs are supposed to be +99 or +100 with archery?

When you have the DX advantage (or you're about even), yes.
If you have DX disadvantage and enemy has enough evasion, CTH can actually drop to (near) zero.
