I was wondering what I would have to add to dexterity or the + on my katana to obtain at least a small chance of a triple hit. I currently have my katana at +20 but with my Menelvegorian Cape [0] (+5) and Pair of Gloves of Mercy [0] (+8) it has an effect of +41. I always get double hits and my dexterity is 5,003. Thanks in advance!

The MenC and GoM have no affect on your katana.
With your kat, you need it to be +41 with a full dex to have 1% chance for a triple hit. Not a very good chance, but still a chance.
Max DX advantage = 160 CTH, so +41 gives you exactly 1% chance to triple hit.

No, for a katana it's when you have 2 and 2/3 as much DX as your opponent.

So it's 1 + CTH times the dex of your opponent?
Almaisky, not sure what you mean. For every weapon the max cth is 100% + bth. So for 1h it is 100% + 60% or 160%. To find your final cth before pth take your DX over your enemies DX. Once your DX is 8/3 then you have a cth of 160% and it's as high as you can go. A simple way to figure this out is take 160%/60%. Or if you have a 2h weapon take 140%/40%.
