Create me an email form page (in Contests)

I am in need of a simple page, in which a player / person can type some text, along with a (short) title and click on a button to email me at Shade@cbstuff.nl.

CSS is also to be found on cbstuff.nl if you use it.

prize will be $25k most likely, don't know how hard it is :)

That page should allow you to create a servicable form, provided you have php access.

on a side note, Pixel Sage gave me the url of a website (w3 schools) that explained it all, silly me to be so lame not to check it first :p

nms' version of formmail.pl is what I would recommend. Writing a mail handler yourself will result in something that's way too easy to be exploited by spammers.
