Something about that really looks fake. I think maybe how low the top teeth come down, and the lack of an inner rim on the ring.
NS it is a convex spacer/ring. One of my best friends is a tattooist/body piercer and I have seen him sell/insert these before. This one is a temporary (pops out), but I have seen permanents (sown in/ implant type) for the lip, cheek, skull, leg and arm. I am sure there is more but I have not seen them, they kinda creep me out.
